I acknowledge that there are potential risks associated with undergoing a lash lift and tint service on my natural lashes and/or brows, albeit rare. While the occurrence of eye irritation and discomfort is infrequent, I am aware that such possibilities exist.

I acknowledge and give consent to having my eyes closed and covered for a duration of 1-2 hours throughout the service. The duration may vary depending on the specific service being provided.

Current use of eye drops of any kind, prescription or over-the-counter?

Please list current allergies

History or recurrent eye or tear duct infections?

History of dry eyes or other medical conditions that would prevent me from getting this service?

I have NOT had a lash lift performed on my lashes in the last 4 weeks.

I agree to the following maintenance instructions to ensure the retention of my lash lift.

Avoid any water contact with the eye area for 24 hours following the application.

I will refrain from applying mascara for 12-24 hours after the service.
